Important Note

  1. Compliance with Senate Bill 205 does not affect all businesses, but all business are required to submit the Stormwater Discharge Compliance Form prior to renewing or applying for a Business License in the City of Modesto.

Applications and Agreements

All Business License applications are fillable PDFs. When you open the form, click in the first field and type in the information to complete the form. Print the application and click on "Save a Copy" to keep for your records.

Tips for Submitting your Application or Agreement

  • Read your information thoroughly to make sure you are submitting a complete and correct application.
  • Fill out the application(s) completely. Please be sure that the business start date and owner information is filled in.
  • Sign the Business License application.
  • Make sure your payment, payable to City of Modesto, is enclosed for the appropriate Business License tax (fee).  We cannot process incomplete applications or applications returned with incorrect Business License tax (fee).
  • Mail completed application and payment to:  City of Modesto, PO Box 3442, Modesto, California 95353.
